HOW TO COMPLETE AND SUBMIT THE CONSENT WAIVER:
1. FILL OUT THE INFORMATION NEEDED ON OUR WEBSITE
(FIRST NAME, LAST NAME, EMAIL, & PHONE NUMBER)
2. ACCESS AND SELECT THE CONSENT WAIVER YOU
NEED BY CLICKING THE "DOWNLOAD WAIVERS" BUTTON
ON THE WEBSITE
3. DOWNLOAD, READ, FILL OUT, AND SIGN THE WAIVER

For mobile users: The file will be saved in your Downloads folder or Files app.

For desktop users: The file will be saved to your Downloads folder or a location of your choice.
4. PREPARE TO UPLOAD THE SIGNED CONSENT WAIVER

If you completed the waiver on paper, take a clear screenshot or photo of the second page where the signature is located.
5. UPLOAD THE SIGNED WAIVER AND CLICK SUBMIT

Go back to the main website. Under the SERVICES page, look for Consent Waivers and upload a screenshot/photo of your signed waiver by clicking the "UPLOAD WAIVER" button.

Check or tick the box, then click SUBMIT.
